Insurance for South Orange County Patients

INSURANCE

We accept all Private/Commercial insurances, Medicare, Medi-Medi Workers’ Comp claims, Auto Insurances, Memorial Healthcare*, Prospect Medical*, Personal Injury Liens (selective) & Self-pay.

INSURANCE VERIFICATION:

As a courtesy to our patients, we will contact your insurance company to verify your benefits prior to making your first appointment. Should there be a discrepancy on benefit details reported, we encourage patients to also contact their insurance company for better understanding of their out-patient physical therapy benefits.

We accept all PPO plans including, but not limited to:

  • Aetna
  • Blue Shield of California
  • Cigna
  • Great West
  • Tricare
  • United Health Care
  • Anthem Blue Cross
  • Blue Cross /Blue Shield (out of state)
  • Memorial Care (HMO)
  • Health Net
  • Triwest
  • Monarch (Pelvic Only)

All patients with PPO plans are invited to call us even if their insurance is not part of the list, as we can verify their coverage prior to making an appointment.

Upon Verification, patients are informed regarding:

  1. Need for medical prescription for PT; if needed then patient must provide a prescription for the insurance to pay.
  2. Deductible payments (patient’s responsibility)
  3. Co-insurance or co-payments per visit (Co-pays will be collected at the time of the visit)

PAYMENT OPTIONS:

We accept Visa, Mastercard, cash and personal checks.

*Please note that there is a fee for a returned check due to insufficient funds.

CASH PAY PATIENTS:

Cash pay patients are welcome. In California, a patient paying cash without using insurance, can be evaluated and and treated by a Physical Therapist without a physician prescription. We will gladly accpet any prescription you have for therapy, but you do not need one if you are paying cash without insurance involvement.
